Preparing For Your Visit

Aloha! We look forward to seeing you. Please bring the following to your appointment:

Cancel/Rescheduling An Appointment

Please give us at least 48-hour notice if you need to cancel or re-schedule an appointment in order to allow someone else to utilize the appointment and to avoid a no-show fee ($40 for clinic appointments and $100 for surgery and Mohs appointments). Please do not hesitate to reach out to us if you have any questions prior to your appointment at 808-888-3311.

Children & Minors

Children are not allowed in surgical or procedure appointments unless they are the patient. Children are also not allowed to wait in the waiting room unaccompanied. All children 17 years of age and younger must be accompanied by a parent or guardian.
 
FOR MINORS UNDER THE AGE OF 18: The parent or guardian must sign all necessary registration consent forms. These are available on the patient portal, SADIO, and can be done ahead of time. They can also be signed on any electronic device when confirming the appointment through our automated text reminder system.
 
*If a parent or guardian will not be coming to the appointment, a consent form giving permission for the patient to be seen alone or to be seen with someone other than the parent or guardian must be signed prior to the appointment. Please contact our office as soon as possible in order to sign all necessary consent forms BEFORE the patient is seen or they will need to be rescheduled.

All co-pays, deductibles, and co-insurances for the estimated office visit are collected at the time of the visit. Additional procedure costs, if applicable, will be collected at the time of the procedure. We accept all major credit and debit cards, cash, and check.
